The Calea Nursing team can support you (and your patients) with:
Training for new HPS patients – the Calea team have capacity available to support our HPS patients in their transition from hospital to home, whether they are commencing their training or continuing progress made in hospital.
Ongoing nursing for HPS patients – We have good capacity for the ongoing nursing of HPS patients, for connections, disconnections and dressing changes, having accepted 97% of referrals so far this year. Contact the Calea team today to find out about capacity in your area.
Respite nursing for HPS patients – respite nursing for self-caring patients/families for example in times of illness or injury.
Review visit for self-caring HPS patients - opportunity to review clinical practices such as technique, line management, hygiene in addition to management of stock levels and repeating the home risk assessment. Now reimbursed under the National HPN Framework Agreement for England,
Transition training for patients transitioning from paediatric to adult care - to ensure they are fully trained to self-care by a Healthcare Professional in their home setting.
Rapid discharge for palliative patients – our nursing and wider Calea team can support accelerated discharge for patients prescribed a licensed multi-chamber bag (MCB) to ensure that palliative patients have the maximum time at home with their loved ones possible.
