The BAPEN Medical Committee has been busy organising of our Annual Study Day (the day before the main BAPEN Conference on Monday 20th November 2017). The title: ‘Intestinal Failure through the Ages’. This year, the Study Day is being run in conjunction with BIFA and BSPGHAN. We are going to cover a range of topics relating to intestinal failure from both an adult and paediatric point of view.

It is clear that while there is overlap between adult and paediatric care, there are also differences not only in physiology but also practical care. The day will not only provide a state of the art update on intestinal failure management but also the ability for practitioners for age ranges to be exposed to best practice of the other. Topics will be covered using formal talks and interactive case presentations by a variety of doctors, dietitians and nurse specialists within the field. I am sure the day will be a great success.

These first few months of the year have been exceptionally busyl as I am sure we can all appreciate!

The NNNG welcomed two new members: Natalie Welsh joins us from Tameside & Glossop Integrated Care NHS Foundation Trust and Krista Murray from University College London Hospitals. The work of the NNNG continues to grow and so having new people to join when existing members come to the end of their term is essential. Our newest Committee member Natalie was placed third in the recent British Journal of Nursing Awards, Nutrition Nurse of the Year. This year saw the highest number of nominations for the Nutrition Nurse of the Year award and highlights the importance of the nutrition nurse role and also the diversity within it.

In April, the NNNG launched its latest set of Good Practice Guidelines. The guidelines are a significant part of the work carried out by the Committee and its members. Their formation helps to provide a resource for all those providing nutritional care, enhances training and improves patient safety. This month also sees the launch of our Twitter account. This will enable us to spread the work of the NNNG and also link and engage with like-minded professionals. You can follow us at @NNNGUK.

Once again, the NNNG were represented at this year’s London Nurse Show. This was a very busy event attracting delegates from all aspects of nursing. The NNNG ran two sessions: Safe insertion of nasogastric tubes and ongoing care and Not all PEGs are PEGS! Each of the sessions were well attended, with over 40 delegates per session. As well as the seminars, we exhibited the NNNG stand and were able to recruit new members and promote the work of the NNNG.

And, finally, July 10th & 11th are the dates for the 2017 NNNG Conference in Bournemouth. The programme for this year is both exciting and diverse and is a must for not only nurses but other professionals involved in the care of patients with complex nutritional needs. With topics such as Ehlers Danlos, nutrition in COPD and a specialist paediatric symposium book your space now at this essential conference.

Meetings 2016

The members of the NIFWG (Nutrition and Intestinal Failure Working Group) met in January at the annual meeting and on 1st December prior to the intestinal failure and rehabilitation study day in Newcastle.

There were two meetings jointly held with BAPEN. The BAPEN paediatric day took place on 8th November at the 2016 Annual Conference in Brighton and focused on nutrition and gut function and hospital nutrition support. The third paediatric intestinal failure and rehabilitation meeting organised by Julian Thomas was held in Newcastle on 2nd December 2016. The programme was wide spread and included talks about immune dysregulation of the gut, a consensus approach to decision making in long-term PN for children with neurodisability, non transplant surgery in IF and outcome of small intestinal transplantation in UK children, motility disorders, and the current state of Paed e-BANS. Blenderised diets and child protection concerns amongst children on home PN were discussed in workshops.

Paediatric e-BANS

A major focus of the group in 2016 was to ensure that the paediatric section of e-BANS (Paediatric electronic-British Artificial Nutrition Survey) will continue to be accessed by the intestinal failure units and all relevant patients are captured. Andrew Barclay who continued to lead on Paediatric e-BANS gave an update at the Intestinal Failure and Rehabilitation Study day in Newcastle and summarised what has been achieved to date. Andy stepped down from his role as Paed e-BANS chair at the end of the 2016 and has written a report and made recommendations what the next chair should take forward. The NIFGW would like to express profound thanks to Andy for his hard work and achievements.

Akshay Batra took over the role as Paediatric e-BANS Chair from Andrew Barclay in January 2017. In order to share the significant workload, the structure of Paediatric e-BANS has been changed. From now on the Chair will be supported by a Secretary (Julian Thomas) and Regional Deputy Lead (Tony Wiskin). A dedicated deputy for each region has been chosen to make sure that data collection is complete.

Meetings 2017

The members of the NIFWG are planning to meet three times in 2017.

A joint symposium on intestinal failure to be held both for paediatric and adult health professionals is planned to take place at the 2017 Annual BAPEN Conference in Birmingham on 20th November. The draft programme looks exciting.

BIFA

Theo Wong has taken over from Sue Protheroe to represent BSGPHAN. The BIFA Position Statement (2016) on Home Parenteral Nutrition (HPN) has a reference to children and is published online.

The group continues to focus on a number of projects, including:

  • Guidelines for the diagnosis and management of intestinal failure in childhood
  • The development of a decision making tree for feeding neuro-disabled children with deteriorating intestinal function in conjunction with the Royal College of Paediatrics
  • To obtain funding for a randomised controlled trial investigating the use of Taurolock prophylaxis for the prevention of central venous catheter associated sepsis
  • To develop a practical ethical framework for decision making in severe neonatal intestinal failure
  • To establish guidelines for the use of blenderised diets in children
  • To publish recommendations for the best method to wean children from intravenous to enteral nutrition
  • To carry out a national survey on jejunal feeding practices in children across the UK.

Following completion the group would like to publish the documents both on the BSGPHAN and BAPEN websites.

The residential element of the PENG Clinical Update Course is only a few weeks away and news about the PENG Award 2017 will soon be announced!

The PENG Pocket Guide to Clinical Nutrition is currently being updated. The updated Pocket Guide to Clinical Nutrition will be available towards the end of 2017 and a complimentary copy will be available for all PENG members during the 2017/18 membership year. The results of the recent survey focusing on the PENG Pocket Guide to Clinical Nutrition was summarised in the recent member's e-PENlines, which will help in the current update of the Pocket Guide.

The Dietetic Outcomes Toolkit ('DOT') continues to evolve, and more news will follow in the coming months.

We continue to offer a PENG Grant Award, which is available to PENG members to apply for through the PENG website and Jacklyn Jones (Research and Audit lead for PENG) is leading on this.

More recently, some exciting opportunities have arisen for PENG members (and BDA members) working in the field of nutrition support (for example, those working in intestinal failure [IF]). Firstly, NHS England has asked members to come forward to form a working group, along with asking dietitians to join their IF network. Kirsten Farrer will now be representing dietitians in the NHS England working group and is going to provide us with regular updates – so watch this space! Secondly, the peri-operative group at the Royal College of Anaesthetists was seeking dietetic representation. Beth Rye from St Mark's will be the dietetic representative moving forward and will provide an update in the next edition of e-PENlines for PENG members. PENG also had a presence at the recent Sheffield Gastroenterology Study Day, which was held on 12th May 2017. Some of the Committee played an active part in the day as key speakers, and it also provided the opportunity to share information on PENG resources with delegates.

  • Alison Culkin (PENG PN and Clinical Update Course Lead) who has become a non-medical Prescriber
  • Vera Todorovic who has been shortlisted for the 2017 CN 'Outstanding Achievement' Award
  • Sean White, who with others, developed the fantastic MND YouTube site which has been shortlisted for the 2017 CN Award for Resource of the Year.
